We need help as soon as you are available! We are looking for qualified RN's for case managers to do initial home health evaluations, post hospital evaluations and discharge visits. Will require being on call once a month to perform after hour IV therapy admissions if needed. Must have IV therapy and wound vac experience. Must be able to speak English and Spanish. We provide a laptop for use. Current need is in the west El Paso, Northeast, and the central part of town. Must turn in documentation daily. Job responsibilities are the following:

1.RN Demonstrates clinical competency in areas of assigned nursing responsibilities.
2. RN Performs treatment, procedures, teaching and training to include but not limited to:
a. Providing skilled nursing care, including advanced assessment and interventions, in the patient’s home as ordered by the attending physician(s) in conjunction with the plan of care.
b. Conducting basic physical and mental status assessments; assists patients in filling out various forms; records health history and assesses medical status; establishes and documents a health care plan under the supervision of a physician, and Director of Nursing; provides instruction and education to patients for various health problems and prevention, medical tests and procedures.
c. Performs various routine medical procedures such as giving injections, cleaning wounds and changing dressings; schedules medical tests; administers medications; draws blood and collects specimens and delivers to local labs, arranges for emergency medical care and removal of patient from the home if necessary.
All under the supervision of an Director of Nursing (DON), report significant findings to the physician and the DON
3. Complies with agency policies and procedures; develops, promotes and maintains an agency environment that is in compliance with federal, state and local regulations.
4. RN Communicates information effectively to physicians, supervisors, and others as needed and submits clear and appropriate documentation in a timely manner.
a. The RN will prepare, submit and maintain documentation as required by Altomar Home Healthcare, Inc. on electronic medical records.
b. RN will Assure verbal orders obtained from the physician are accurately transcribed for implementation.
c. Participates in Quality Assessment and Improvement Activities as assigned.
5. RN assures sufficient supplies and medication are available at all times; makes routine inspections of medical equipment and checks daily logs by other disciplines at the home.
6. Utilizes supplies and equipment to enhance productivity and maximize cost controls.
7. Participates in case conference process and ensures implementation of activities for improved quality of patient care.
8. Serves as a clinical resource person and role model for patient care.
EMPLOYMENT STANDARDS:
1. Graduate of accredited school of nursing. RN with valid licensure in the state of Texas.
2. Two (2) years minimum experience is preferred in an acute care setting with experience in
Geriatric and medical/surgical patient care. Some home care experience preferred; supervisory experience desired.
3. Excellent verbal and written communication skills, sound knowledge of patient health, expert technical skills and strong patient teaching skills with the ability to effectively teach patient and facilitate the coordination of care in the home setting.
4. Valid drivers license in the state of Texas, with available and dependable transportation and proof of current automobile liability insurance.
5. Ability to meet minimum agency hiring standards which includes evidence of current cardiopulmonary resuscitation (CPR) certificate, and willingness to perform such procedures in the home setting.

Job Types: Full-time, Contract

Pay: $50.00 - $100.00 per hour
