Job Summary:

The Case Planner (DV Rapid Rehousing/Transitional Housing) supports AFG’s mission, and its goal to rapidly connect individuals and families experience homelessness with permanent sustainable housing.

Key Responsibilities:

• Provide case management and mentoring services to program participants (18-24 years old) to help them obtain and sustain permanent housing.
• Conduct intakes and recertifications to ensure continued eligibility for the program.
• Provide assistance in obtaining and maintaining eligibility for housing choice vouchers.
• Complete check requests and mail rent and utility payments on-time.
• Identify and disseminate community resources to help serve the needs of current and former program participants.
• Help participants develop life skills necessary to sustain permanent housing, such as: obtaining employment, developing housekeeping skills, paying their portion of the rent on-time, etc.
• Compile statistics that contribute to monthly, annual and ad hoc reports.
• Ensure completion of required documentation, and data entry for all program activities.
• Perform other duties, as assigned.

• Ability to handle crisis, cope with job stress and maintain confidentiality

Acquired Knowledge:

• Requires an associate or bachelor’s degree in social work, counseling, psychology, sociology or criminal justice and a minimum of 3-5 years of related experience
• Experience working with youth (18-24 years of age) who are homelessness, mental ill and/or aging out of foster care is strongly preferred
• Familiarity with the Detroit community and its social service resources is needed
• Requires basic computer software and Internet skills

This position requires regular driving. Incumbents must have reliable transportation, must possess a driver’s license and automobile insurance, and must have a good driving record.
