LPN Nurse for Home Health Care Opportunity

Alternative Home Health is seeking a passionate LPN to join our growing team. As an LPN , you would be able to work independently, be dependable, compassionate and have a desire to help others. The Licensed Practical Nurse ( LPN ) will travel to patients' homes and be responsible for administering individualized patient care and perform duties related to hospice and palliative care in accordance with physician orders.

LPN Nurse Responsibilities

• Demonstrate knowledge of personal hygiene techniques.
• Work with a team and implement nursing care plan for each patient.
• Communicate effectively with patients, families, physicians, and other healthcare providers.
• LPN provides physical care to patients in a home- or community-based setting.
• Assess patient's physical and mental status; initiate preventative and rehabilitative nursing procuedures as appropriate.
• Observes signs and symptoms and reports to the physician and LPN reactions to treatments, including drugs and changes in the patient's physical or emotional conidition.
• Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
• LPN maintains accurate and complete recrods of observtions, treatments and care of patient.
• Takes on-call duty rotation; nights, weekends, holidays
• Attends staff meetings, patient care conferences and inservices as scheduled.
