Alternative Home Health is seeking a compassionate LPN to join our growing team. As a LPN, you would be able to work independently, be dependable, compassionate and have a desire to help others. The LPN will travel to patients' homes and be responsible for administering patient care.

LPN Home Health Responsibilities

• Provide skilled services/education for the patient to achieve their optimal level of health while remaining in their home.
• Communicate effectively with patients, families, physicians, and other healthcare providers.
• Assess patient's physical and mental status; initiate preventative and rehabilitative nursing procedures as appropriate.
• Observes signs and symptoms and reports to the physician reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
• Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
• Maintains accurate and complete records of observations, treatments and care of patient.
• Takes on-call duty rotation; ranges from every 4th to 5th weekend.
• Attends staff meetings, patient care conferences and in-services as scheduled.
