Aloha Nursing Rehab Centre is a long-term care facility in Kaneohe, HI that operates 24 hours a day/7 days a week. We are looking for an experienced Social Services Administrative Assistant with excellent communication skills, who is efficient and well organized to join our compassionate Social Services team.

ESSENTIAL JOB FUNCTIONS:

• Ability to effectively communicate with patients, families, responsible parties, staff and outside resources and agencies.
• Assist in the coordination of patients’ discharges from the facility, and provide for the acquisition of necessary medical equipment and services.
• Provide applicable notices, including but not limited to, discharge and appeal rights to facility residents and/or the residents’ representatives, and follow up on residents’ appeals as necessary.
• Ensures completion of discharge paperwork/activities, i.e., review of discharge checklist, follow-up with appropriate disciplines and photocopying of documentation.
• Contacts appropriate community resources and coordinates services, i.e., home health, transportation, as needed according to the Social Worker’s discharge plan.
• Orders durable medical equipment as needed according to the discharge plan.
• Conducts follow-up phone calls to residents after discharge and generates a monthly report of findings.
• Assists in creating Behavior Care Plans under the direction of the Social Services Manager and Social Worker.
• Conducts quarterly Advance Directive Audit/POLST audit as directed by the Social Services Manager.
• Assist residents/families in completing various forms and applications, including advance directives, Medicaid applications, etc.
• Be knowledgeable in the use of Microsoft Office applications.
• Full-Time; Monday- Friday; 8:00 am to 4:30 pm

Qualification Requirements:

• Requires good communication skills.
• Must be able to exercise judgment where procedures cannot be standardized and seek assistance as necessary.
• Must have an understanding of the social and psychological needs of the aged as well as other types of residents.

Education/Training: B.S. or B.A. degree preferred; high school diploma required.

Experience: Working in a long-term care facility preferred.

Must meet the basic health requirements including 2 step TB skin test or chest x-ray
High school diploma or equivalent
