Immediate Job Opening for a Targeted Case Manager with Allheart Senior Care for the Batesville, AR office. Will serve multiple counties.
Must have two years’ experience in the delivery of human services, including without limitation having performed satisfactorily as a case manager for a period of two years (experience must be withing the past three years): or hold a license in the state of Arkansas as a social worker (Licensed Master Social Worker or Licensed Certified Social Worker), a registered nurse or a licensed practical nurse; or have a bachelor’s degree from an accredited institution in a health and human services or related field.
Starting Wage is $18 - $20 per hour depending on knowledge and experience.
Job requires traveling to client’s homes at least quarterly, identifying needs and services, making appropriate referrals, and performing monthly monitoring of progress by phone. Transportation is provided: must have valid Arkansas driver’s license with a clean driving record. Secondary: is cultivating/procuring new clients, educating community stakeholders, and completing eligibility documentation.
Job will start with 20-40 hours in the Batesville office, depending on demonstrated skill and ability.
