Freedom Behavioral Hospital of Plainview, Texas is a 20-bed, acute geriatric psych facility located in Plainview, Texas. We are looking for an experienced a licensed LCSW, LMSW or a LPC to be part of the Clinical Services Team for our IOP (Intense Outpatient Program( located in New Deal, Texas..
The program hours are 7:00 am to 5:00 pm, Monday thru Friday.
The Counselor will work as an integral part of our social services program, which is necessary to meet the legal, organizational, medical staff and regulatory agency guidelines. The work required would be service and provide therapy services, and education for our patients well-being. All duties to be done in accordance with Joint Commission, Federal and State regulations, Allegiance and Freedom policies and procedures and Performance Improvement Standards.
Essential Functions :
Provides clinical supervision of individual or group session for patients as indicated by the providers orders.
Participate as a member of the multi-disciplinary treatment team and participates in chart reviews for quality assurance.
Work with the clinical department including the following functions: discharge and after care planning, completion of discharge summaries, completion of clinical documentation and providing guidance to the clinical staff on needs of our patients upon discharge from the inpatient program for referral to the outpatient program.
Review psycho-social documentation, completion of outcome studies assessments and development of clinical notes.
Establishes and maintains a supportive/directive relationship to foster a positive organizational culture making self available to staff.
Provides clinical counseling services; including group or individual and family services including communication in an age/disability/cultural sensitive milieu.
Performs other duties and projects as assigned.
Educational/Experience Requirements:
Must be licensed in the state of Texas as a License Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW) and or Licensed Master Social Worker (LMSW)

Requirements:
Qualifications/Skills: Excellent assertive communication skills. A sound individual who possesses excellent communication and interpersonal skills to foster cooperation across departmental lines and manage information, environmental and fiscal resources to accomplish goals. Good writing and composition skills. Able to organize and prioritize high volume workload. Able to function with minimal supervision. Therapeutic Intervention Ability to maintain overall good work attitude and interact cooperatively and professionally with other staff members. Basic competency in age/disability/cultural diversity needs of patients served and ability to relate to patients in a manner sensitive to those needs. Must be able to work under close supervision. Must successfully complete CPR certification as well attend our approved behavioral de-escalation program.
This position will consist of chart documentation, treatment planning, performing psycho-social assessments, and patient groups.
