Qualifications

• Licensed in the state of Pennsylvania prior to employment
• Unrestricted DEA certificate
• PhD/MD Graduate of accredited medical school with degree of Doctor of Medicine (MD) or Doctor of Osteopathy (DO)
• Completion of accredited residency and fellowship by specialty
