Job Description :

GENERAL OVERVIEW:

Facilitates referrals of rehab patients to the Inpatient Rehabilitation Program as appropriate by establishing relationships with a variety of hospitals, departments, providers, payor organizations, physician offices, post-acute facilities and community agencies. Evaluates referred patients to assess their appropriateness for admission to an inpatient rehab program. Facilitates admission working with the rehab attending physician and the nursing team. Develops and implements a marketing/community relations plan for the rehabilitation program in conjunction with System Inpatient Rehab Programs. Functions as a liaison between community physicians, their staff and system rehab facilities.

ESSENTIAL RESPONSIBILITIES:

  • Provides education about services, facilities and resources of the inpatient rehabilitation program to a variety of hospitals, departments, providers, payor organizations, physician offices, post-acute facilities and community agencies. (15%)
  • Conducts onsite assessments and identifies potential rehabilitation candidates for appropriateness and medical stability for admission to the inpatient rehabilitation program. (15%)
  • Facilitates the transfer of appropriate clinical information to appropriate individuals in order for the physician to review/approve the admission and for clinical staff to prepare to care for the patient. This includes extensive interaction with physicians, nurses and ancillary staff to facilitate improved clinical documentation. Provides freedom of choice to patients/families in acute settings to identify post-acute needs. (10%)
  • Completes clinical documentation and other paperwork related to the patient admission accurately and within required timeframes. (10%)
  • Works with discharge planning, case management staff and the system inpatient rehab program to facilitate a smooth transition for the patient and family into rehab. (10%)
  • In cooperation with other relevant stakeholders, develops and implements the clinical marketing plan for the inpatient rehab program. (10%)
  • Solicits feedback from community physicians and their office staff in order to improve their experience with the inpatient rehab program. (10%)

  • Works with the Executive team to address concerns of community based physicians and their staff. (10%)
  • Other duties as assigned.

QUALIFICATIONS:

Minimum

  • Bachelor’s degree in Nursing or related field OR 6 years of related experience in lieu of Bachelor's Degree OR similar combination of education and experience
  • 3 years in physical rehabilitation or acute care setting
  • Marketing experience

Preferred

  • 1 year experience in Medicare/Medicaid/commercial insurance
  • Registered Nurse (PA license), Physical Therapist or Licensed Social Worker
