GENERAL SUMMARY:
Ensures the delivery of quality nursing care, evaluates individual status, directs/supervises ancillary staff, and adjusts nursing care priorities. Acts as a role model and preceptor to new or less experienced staff members is knowledgeable regarding standard facility policies, operating procedures, and regulatory guidelines.
FUNCTION OF JOB:
1. Oversees and/or provides care to individuals utilizing prescribed long-term care treatment modalities.
2. Monitors the nursing care provided by the certified nurse's aide and certified medication aides assigned to the home during each shift and provides clinical direction and assistance as appropriate.
3. Assigns each staff member tasks and individual care responsibilities in accordance with the staff member's level of education and clinical preparation.
4. Interacts with attending physicians and makes recommendations for changes in care based on results of nursing assessments. Initiates physician's orders and appropriate nursing interventions as needed.
5. Actively participates in staff education programs. Identifies program topics.
6. Conducts informational educational classes as the need arises, in order to orient staff to new equipment, procedures and treatment.
7. Acts as a resource nurse, a role model, and a preceptor for other nursing staff.
8. Participates with other allied health disciplines in individual care conference aimed at meeting the needs of the total individual and family.
9. Identifies, resolves, reports and records unusual incidents or occurrences in accordance with standard operating procedures.
10. Assess staff performance levels and communications findings to program directors.
11. Operates facility equipment safely and effectively and is knowledgeable regarding standard procedures for maintenance of nursing equipment
12. Document all nursing interventions/nursing care in writing and within Appropriate time frame
13. Develops an admission assessment or updates current individual care plan for Each individual in the home
14. Understands, supports, and enforces all facility and regulatory agency policies and procedures
15. Oversees the ongoing maintenance of a clean, safe environment for individuals and staff. Initiates corrective action as appropriate.
16. Dispenses prescribed medications, maintains appropriate security of all controlled substances, and performs nursing care within the established scope of nursing practice.
17. Interacts and communicates with individuals’ family members, co-workers, physicians, and general public in a courteous and professional manner. Notifies physicians, program director and family member(s) of significant changes in the Individuals condition.
18. Gives/receives report upon reporting in and ending of shift.
19. Work with the Office of Developmental Programs (ODP)
EDUCATION, EXPERIENCE AND LICENSURE:
1. Graduate of an accredited RN program
2. License in the state of Pennsylvania
3. Must complete State Criminal Background check.
4. At least one (1) year of experience in a Group Home or Long-Term care.
5. Must have experience working with individuals with development disabilities and or Autism.
