Description of the Services

Consultant shall provide to Company the following services (including, as applicable, preparatory work and revisions), in exchange for payment as provided for under the Agreement:

• Provides counseling of individuals and families

• Work in a telehealth Intensive Outpatient Setting.

• Facilitate psycho-educational, experiential and process groups when indicated.

• Willingness to get licensed in multiple states, participation in this process with different departments

• Maintain up to date and accurate progress and incident notes on clients in EMR system.

• Conduct psychosocial evaluations, create individualized treatment plans, ongoing treatment plan reviews and discharge summaries for clients when indicated.

• Collaborate with clinical team as needed for client care

• Provide therapeutic interventions (e.g., EMDR sessions) to individuals, groups and families as needed.

Consultant acknowledges and agrees that Consultant shall at all times:

• Attend and participate in all orientations and trainings required by the Company, for which time Consultant shall be paid the rate stated on Exhibit B

• Maintain the ability to tailor treatment strategies to the patient’s stage of dependence, change and recovery.

• Demonstrate an understanding of the principles of addiction and DSM-5 care systems.

• Demonstrate an understanding of the role Developmental trauma plays in addiction and other behavioral disorders.

• Demonstrate an understanding of the psychological, behavioral, physical health and social effects of psychoactive substances and behavioral disorders on patients.

• Demonstrate knowledge of models and theories for treatment of Trauma, Addiction and Mental Health Disorders.

• Perform Clinical assessments, consultation, referral and education according to the APN Treatment models.

• Understand the established diagnostic criteria for substance use/behavioral disorders treatment modalities and placement criteria.

• Understand the importance of research and outcome data in clinical practice while using APN measurement systems.

• Understand the value of an interdisciplinary approach to treatment and work collaboratively with all APN departments.
