The Registered Dietitian is responsible for overall menu planning and approval. Responsible for resident assessment, intervention, communication, and implementation of all dietary recommendations. Responsible for coordination of dietary interventions with health practitioners and departments at the Forum.

Employment Type: Part Time 

Wage Range: $41-$55 per hour

Here are a few of the daily responsibilities of a Cook:

  • Assists in developing all menus for Health Center residents assessing preferences and nutritional needs. Assists in assuring staff education is provided to dietary personnel in accordance with the staff education plan plus provides on the job training for Health Center dietary personnel.
  • Assures that each resident’s nutritional adequacy is met by utilizing the assurance of Nutritional Adequacy Policy.
  • Maintains pertinent and appropriate records, reports, schedules and studies as required by local, state and federal regulations and otherwise requested.
  • Initiates and supervises the completion of hands-on intakes as assigned.
  • Maintains individual diet information by transcribing information from the dietitian’s weekly notes to the tray card, menu and nourishment list when indicated.
  • Attends Care Plan Meeting.
  • Interviews new admissions and documents food preferences on the approved form.
  • Contacts the dietitian between scheduled visits if unplanned or special therapeutic diets are ordered.
  • Participates in scheduled audits
  • Completes MDS, assessments, care planning as required
  • Participates in weight and skin meeting, QAPI meetings and other meetings as assigned
  • Performs timely assessments and recommendations related to weight loss and skin issues.
  • Assists with special diets and menus for all residents in all levels of care at the Forum as needed.
  • Provides dietary education and assistance for all residents in all levels of care as needed.

Here are a few of the qualifications we need you to have:

  • Bachelor of Degree in Nutrition 
  • Must be a registered dietitian or hold a C.D.M. certification in healthcare. 
  • A minimum of two years supervisory experience in food service operations of a health care setting. 
  • Certified in safety and sanitation regarding food service. 
  • Prefer to experience in a long-term health care environment. 
  • Experience with performing clinical assessments on residents, meeting with Care Plan Team, performing input on MDS. 
  • Ability to communicate with, understand, empathize with, work with and be tolerant of residents, families, visitors, peers, supervisors, and subordinates. A
  • Ability to interpret and explain menus, refer, or answer questions, coordinate meal service process and understand requests.
