All Care VNA has a full-time position available for a Clinical Intake/Referral Specialist (RN, PT, PTA or LPN). This position is Monday thru Friday, working in our state of the art office 4 days a week and working 1 day remote.

In this role you are the primary contact for referrals sources via Electronic platforms, fax, phone or from our Business Development Liaisons. You will review and accept Clinical referrals for all services lines and case conference with clinical managers and liaisons regarding complex care cases.

You will assist in the collection of clinical information to accurately complete referral documentation/ and to help facilitate the admission process. Must have knowledge of medical terminology and at least 1 year of working in a Home(add) healthcare environment preferred. Excellent Customer Service Skills, data entry skills, ability to mult- task and Proficient computer skills.
