As an RN Case Manager in community health, you can make a life-changing impact on people from all walks of life. Work one-on-one with your patients in the comfort of where they call home. This allows you to develop deeper relationships and work in a more personal setting than a traditional medical office. Additionally, you will practice with more independence and autonomy than in traditional nursing roles, with flexibility in managing your own schedule full or part time.

If you enjoy providing wound care, IV/ infusions, cardiopulmonary assessment, diabetic teaching, post-surgical care, disease management, medication reconciliation and management, gastrostomy tube care, injections, blood draws, in-dwelling catheter care and management; and overseeing patient care while collaborating with providers and an interdisciplinary team.
