Our agency is looking for dedicated, caring, and experienced Sponsored Residential Providers willing to provide a loving home for adults diagnosed with developmental disabilities.

**This position requires the individual to live in your home.**

Responsibilities:

• Provide care and support to individuals with developmental disabilities in a home-based setting.
• Ensure the daily needs of the individual(s) are properly met.
• Provide direct supervision and guidance to ensure the safety and welfare of individuals.
• Follows established procedures to report changes/concerns to appropriate persons.
• Complete data collection and documentation in accordance with the Individual Service Plan.
• Participates in staff meetings and in-service training as assigned.
• Plans and implements community activities.
• Protect individuals from any form of abuse, neglect, and exploitation
• Schedule and attend appointments as needed with physicians and other providers.
• Cooperates with any investigation, meeting, hearing, or appeal held under these regulations. Cooperation includes, but not limited to, giving statements or sworn testimony.

Qualifications:

• Minimum 1 year of experience in working with individuals with developmental disabilities (Direct Support Professional-DSP)
• Strong communication and interpersonal skills
• First aid and CPR certification is required
• Must be able to work independently and as part of a team
• Must have a high school diploma or equivalent
• Must have a valid driver's license and a clean driving record
• Own a safe and reliable vehicle
• Own or lease your home
• Criminal background free of barrier crimes
• Complete required training
• Cooperate with guidelines and provide detailed information for your home to become licensed
• Ability to complete required documentation using computer software

Job Type: Contract
