Job Description:
We are seeking a dedicated and experienced Advanced Practice Provider (APP), either a Nurse Practitioner or Physician Assistant, to join our team in providing exceptional care within the Post-Acute Care setting. This position plays a crucial role in our mission to deliver innovative senior care solutions across our network of over 1000 senior living facilities in 26 states. The ideal candidate will embrace our philosophy of care, focusing on the maintenance or enhancement of each patient's clinical status and quality of life.
Primary Duties and Responsibilities:
- Conduct Annual Wellness Visits and potentially Annual History and Physicals for long-term nursing facility residents.
- Perform clinical interventional and episodic direct care visits, along with appropriate re-evaluation of care visits.
- Offer ongoing evaluation and treatment of chronic, co-morbid illnesses.
- Evaluate and treat residents for changes in condition, reviewing medications, orders, and diagnostic tests.
- Coordinate care with collaborating physicians for overall medical management.
- Assist with the management of facility-wide illnesses and coordinate care with other disciplines.
- Maintain communication with families and attend family conferences whenever possible.
Qualifications:
- Master's degree or higher in Nursing.
- Valid Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of Pennsylvania.
- Current board certification in your field.
- Demonstrated critical thinking skills, with experience in nursing required.
- Experience as a Nurse Practitioner/Provider preferred.
- Strong leadership, interpersonal, and problem-solving abilities.
- Excellent written and oral communication skills.
- Fully vaccinated against COVID-19 in compliance with EEOC guidance.
