POSITION TITLE:
CASE MANAGER: EARLY WARNING - THREAT ASSESSMENT - Short Term
JOB DESCRIPTION:

Supports families, school staff and community partners in their efforts to assist students in achieving educational excellence and bridge the gap between home, school and community.

ESSENTIAL FUNCTIONS:
  • Meet with families and students to determine needs that are causing barriers to student learning (including attendance and behavior issues).
  • Promote, access, track, analyze, and utilize various student, family, school and community data indicators to target resources and interventions and monitor progress.
  • Documents and tracks work tasks, functions, progress and outcomes.
  • Establish and maintain communication with families regarding student academic, attendance and behavioral progress.
  • Conduct root cause analysis and provide case management services to support and optimize student success in the educational setting.
  • Collaborate with multiple and varied individuals, school committees, and agencies regarding services, opportunities and supports for families and students.
  • Assume other duties and perform other tasks as assigned by immediate supervisor.

FOCUS AREA:

  • Early Warning System
    • Align and track outreach, intervention and referral efforts with district and school teams such as School Health and Wellness Team
    • Coordinate with Early Warning System Team
    • Coordinate with Student Assistance Team
    • Coordinate with Attendance Team
    • Support school staff to establish aligned and effective systems and processes.
  • Foster Care
    • Align and track outreach, intervention and referral efforts with district and school teams such as School Health and Wellness Team
    • Coordinate with district Foster Care Unit
    • Work in APS Student Information System Synergy
    • Support school staff to establish aligned and effective systems and processes.
  • Threat Assessment
    • Align and track outreach, intervention and referral efforts with district and school teams such as School Health and Wellness Team
    • Coordinate with district Threat Assessment team
    • Collaborate with Threat Assessment Psychologist
    • Support school staff to establish aligned and effective systems and processes.
PREFERRED KNOWLEDGE, SKILLS, ABILITIES AND EXPERIENCE:
  • Certification and/or experience in conducting case management assessment, planning, referral, monitoring, etc.
  • Skilled at the development of SMART goals.
  • Knowledge of school, district and/or community resources, services and supports.
  • Effective communication skills, both verbal and written.
  • Flexibility, decision-making and problem solving skills.
  • Experience with data analysis.
  • Bilingual
  • Experience helping families access the social service, health, and/or educational system
  • Experience accessing and/or strengthening a network of families and/or service providers
REQUIREMENTS:
  • Bachelor’s degree and five (5) years of experience in education, health, social work, psychology, counseling, or related field OR Associate’s degree and nine (9) years of experience in education, health, social work, psychology, counseling, or related field OR High School Diploma and twelve (12) years of experience in education, health, social work, psychology, counseling, or related field
  • Experience in conducting assessment, planning, implementation and monitoring processes.
