Albany County Department for Children, Youth and Families is Hiring a Family Services Specialist Near Albany, NY

FAMILY SERVICES SPECIALIST

DISTINGUISHING FEATURES OF THE CLASS: This employee provides assistance and direction to families with children in need of a wide range of special services for disabilities. The incumbent in this class is responsible for identifying and coordinating needed services as shown by case management plans on individual children. Work is performed under the supervision of the Early Intervention Service Manager. Does related work as required.

TYPICAL WORK ACTIVITIES:

  • Identifies the need for individual care with the evaluator and members of the family;
  • Establishes a service treatment plan and determines that the short and long term services needed to assist the client and their families and follows up with service providers and the family to ascertain compliance with the plan is appropriate;
  • Provides with other service providers a comprehensive plan that addresses client total needs including special education;
  • Secures services needed in the plan obtaining providers for continuity of services;
  • May be designated as Early Intervention Official in determining a child’ eligibility and in approving and monitoring appropriate level of services to a child;
  • Assists the client and their family in obtaining services outside the case plan provided through other sources for needs in housing, employment and other social and morale needs;
  • Monitors and evaluates the case management plan received by client to verify the services are given;
  • Works with child and their families to provide alternative services when services are unavailable;
  • Provides assistance to individuals and families to discuss client's progress, treatments and all other related matters focusing on the primary problem;
  • Demonstrates whenever possible basic health care techniques to client/family, such as control lice, lead poisoning problems, proper use of medication and obtaining assistance for social services;
  • Provides direction and guidance to transition from the Early Intervention program to the CPSE program;
  • Keeps and maintains records and prepares reports.

MINIMUM QUALIFICATIONS:

A. Graduation from a regionally accredited or New York State registered college or university with a Bachelor's Degree in Sociology, Human Services, Education or closely related field and two (2) years of experience in public health, education, early childhood or community based program for children/families; OR,

B. License, certification, or registration in one of the following professions: occupational therapy assistant, licensed practical nurse, certified behavior analyst assistant, low vision specialist, vision rehabilitation therapist, physical therapy assistant, physician assistant, registered dietician/certified dietician/nutritionist and four (4) years of experience in public health, education, early childhood or community based program for children/families

SPECIAL REQUIREMENT: At time of appointment and throughout employment:

1. If applicant meets the minimum qualification requiring license, certification or registration (Section B above), then possession of such must be maintained throughout employment.

2. Possession of a valid New York State driver’s license; and successful clearance from the State Central Registry in the New York State Office of Children and Family Services and the State Justice Center for the Protection of People with Special Needs (Background investigations are conducted by the appointing authority).

* note - This will be a provisional appointment until such time as an exam is given. Appointee must take the exam and be reachable on the Civil Service list.
