Alaris Health at Kearny, a 120 bed-skilled nursing Center in Kearny, New Jersey, seeks a social worker to plan and implement social service programs in the Center and coordinate discharge planning. Additional responsibilities include:

• planning and implementing a comprehensive social services program that offers counseling and other support services for residents and their families
• evaluating and being keenly aware of residents medical-related social needs and then formulating the appropriate plan for treatment that fits into their total care plan
• coordinating and implementing family meetings for residents and creating a forum for ongoing discussions between residents, their families/responsible parties and the treatment team
• maintaining residents rights at all times while liaising between all parties involved
• coordinating discharge planning and assisting in the creation of an organized discharge plan for all residents
• and above all, ensuring that residents and their families receive the highest quality of service in a caring and compassionate atmosphere that recognizes the individuals needs and rights

JOB REQUIREMENTS:

• Bachelors degree or higher in social work or human services (knowledge of family assessment and interventions is preferred)
• Certifications required by New Jersey state regulations
• Excellent command of the English language (written, read and spoken), excellent interpersonal skills and knowledge of all governmental regulations relating to the social services department
• Ability to work independently, problem solve and makes decisions as necessary
• Ability to positively interact with everyone from residents to government agencies and the general public
