SUMMARY: Senior Patient Navigator is hired by ARMS to work closely with the Site PI and Women’s Health Services staff to help reduce late-stage breast and cervical cancer by assisting patients with access issues and developing relationships with service providers and tracking interventions and outcomes.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
· Assist Site PI to manage and oversee the breast and cervical cancer screening program
· Ensure program protocols are integrated into clinic workflow
· Ensure accurate consenting, data tracking, documentation and reporting
· Work with Informatics Specialist to identify eligible women and create required reports
· Work with marketing to develop relevant and engaging educational materials and strategies e.g. brochures, signs, e-blasts, texts messages
· Work closely with ARMS Call Center staff to establish a robust in reach communication process e.g. appointment reminders, 2-way texting
· Work closely with and coordinate efforts with existing ARMS Breast and Cervical Cancer Screening programs, projects and grants e.g. ABCCEDP, Sowing the Seeds of Health, Community Foundation, Alabama Power Foundation
· Prepare and write required progress and summary reports e.g. department, supervisor, funder, grants
· Build relationships, facilitate interaction and communication with health care staff and providers (physicians, nurses, care coordinators, patient liaisons, social service staff, diagnostic center staff, tertiary care navigators, and staff
· Identifies patient barriers to care such as transportation, insurance, language, housing and coordinates support or connects patients to community and social support services
· Educates patients on care processes and provides support to reduce anxiety and fear
· Guide patients through the health care system; help patients arrive at scheduled appointments on time and prepared
· Utilize intervention and strategies that are appropriate to the culture, language, age, gender, etc.
· Facilitate referrals for mammogram screenings and other diagnostic tests
· Communicate in a culturally competent manner with patients upon learning they have an abnormal mammogram, biopsy, pap smear, or a positive finding
· Assist patients in finding ways to pay for their breast and cervical health care
· Provide women’s health education to individuals and groups
· Actively participate in internal and external meetings, as required
· Work with Director of Marketing to help develop an outreach plan (year two)
· Other duties as assigned
QUALIFICATION REQUIREMENT:
· Ability to work collaboratively with providers, internal staff and external organizations
· Commitment to addressing health disparities and barriers
· Excellent time management and coordination skills, with the ability to set priorities given multiple work assignments
· High degree of independence, flexibility, initiative, and commitment
· Strong communication skills with diverse populations
· Ability to work flexible hours
SUPERVISORY RESPONSIBILITIES:
· Supervise Patient Navigator
EDUCATION AND EXPERIENCE:
· Associate degree required; Bachelor's degree preferred
· Current LPN or RN License required
· 3-5 years community-based or health center operations experience, preferably a Federally Qualified Health Center
· Program oversight or management experience required
· 1-2 years experience in women’s health care, social services, community health or mental health, a plus
· 1-2 years of supervisory experience preferred
· Bilingual (English/Spanish) a plus
· Proficiency with Microsoft Word, Excel, PowerPoint and Outlook
