Akkase Home Health is Hiring a LPN Near Yankton, SD


The Licensed Practical Nurse will care for ill, injured or convalescing clients in the home health 
setting. They will observe and assess the client for quality of live. Demonstrate individualized 
creativity in educating the patients and or caregiver. Follow nursing policy and procedure per 
agency standards. Follow the plan of care according the physician orders. Demonstrate understanding 
of the state regulations.
ESSENTIAL FUNCTIONS
Essential functions are those tasks, duties, and responsibilities that comprise the means of 
accomplishing the job’s purpose and objectives. Essential functions are critical or fundamental to 
the performance of the job. They are the major functions for which the person in the job is held 
accountable.
• Support quality improvement practices.
• Perform nursing procedures according to agency policy and procedures.
• Administers medications or starts intravenous fluids, noting times and amounts on patients 
charts.
• Observes patients, charting and reporting changes in conditions such as adverse reactions to 
medication or treatment and takes necessary actions.
• Answers patients calls and determines how to assist them.
• Measures and records patients vital signs such as height, weight, temperature, blood pressure, 
pulse, and respiration.
• Provides basic patient care or treatments such as taking temperatures or blood pressures, 
dressing wounds, treating bedsores, giving enemas or douches, rubbing with alcohol, massaging, or 
performing catheterizations.
• Works as part of a health care team to assess patient needs, plans and modifies care, and 
implements interventions.
• Evaluates nursing intervention outcomes, conferring with other health care team 
members.
• Assembles and uses equipment such as catheters, tracheotomy tubes, or oxygen suppliers.
• Records food and fluid intake and output.
• Work under the direction of a RN
• Monitor reactions and patient progress using observation, assessment, and evaluation skills per 
state
• laws and regulations.
• Educate patients and family members according to disease process, medications, POC, 
treatment
• options, and home care procedures according to the plan of care.
• Report to physician and RN of adverse findings
• Follow state regulations.
• Coordinate and monitor patient care and services.
• Comply with HIPPA regulations in and out of the office.
• Follow infection control policy in and out of the office.
• Document skilled visit according to guidelines.
• Maintain patient records according to policy and procedures.
• Participate in in-services, workshops, seminars, and self-study courses annually.
• Required to travel to various geographical locations.
• Other duties as assigned
Required Skills/Abilities:
• Excellent verbal and written communication skills.

• Proficient with Microsoft Office Suite or related software.
EDUCATION & TRAINING REQUIREMENTS
• Licensed Practical Nurse with an active license.
• Minimum of one year of nursing experience as an LPN.
• CPR Certification.
• Must have and maintain current valid Driver's License, and ability to drive assigned vehicles 
with appropriate state license following all laws applicable. Must have and maintain current proof 
of liability auto insurance and remain able to be insured under the organization's fleet insurance 
policy.
ESSENTIAL FUNCTIONS – PHYSICAL & MENTAL REQUIREMENTS

• Requires ability to use up to 50 pounds of force occasionally and/or up to 20 pounds of force 
frequently and/or up to 10 pounds continuously to move objects.
• Work deals mostly with areas such as preparing and reading data and figures, records, reports, 
visual inspection involving small details. Although important, depth perception and field of vision 
(peripheral) are not as critical as the ability to distinguish small details and markings very near 
to the observer.
• Requires ability to grasp, pull, push, carry or otherwise manipulate objects; ability to perform 
tasks requiring action of muscles or groups of muscles and foot and/or hand-eye coordination.
• Requires ability to speak clearly and make self-understood effectively in face-to-face 
interactions; articulate with accuracy to communicate using phone.
• Requires ability to hear and receive verbal instructions correctly, answer phones, communicate 
professionally with people in situations with some background noise.
• Requires ability to concentrate on fine detail with frequent interruption; ability to focus 
attention on tasks for 10-20 minutes at a time on a continuous basis, 20-60 minutes on occasion.
• Requires ability to understand and relate to specific ideas one at a time and to the concepts 
behind specific ideas; ability to remember verbal and written tasks/assignments from a few hours to 
several day period.

WORK ENVIRONMENT
The noise level in the work environment is usually moderate. Exposed to inclement weather when 
traveling to patient homes.
