The Peer Navigator contributes to AHF’s mission by providing client centered syndemic service navigation services focused on improving access and retention in needed HIV care and prevention services in King and Pierce Counties.
The core responsibilities include performing outreach to locate clients, re-engagement of clients, coordinate client navigation plans to ensure clients are appropriately referred and linked to supportive services. The Peer Navigator will link clients to a broad array of services including HIV care, HIV pre-exposure prophylaxis (PrEP) services, HIV/STI/viral hepatitis testing/treatment, case management, harm reduction, gender affirming care, mental health, substance abuse, health benefits navigation, housing, and nutrition services.
The Peer Navigator will utilize their knowledge of the community to build relationships and routinely coordinates with both community partners and other AHF business lines to maximize the delivery of services to those living with and at-risk for HIV infection in a manner that reaches the greatest number of individuals while executing AHF’s core values.
