The AIDS Center of Queens County, Inc. (ACQC) has a strong commitment to providing HIV/AIDS prevention, care and advocacy for individuals throughout the borough of Queens and beyond. A leading provider of services since 1986 for those who are HIV and at risk, ACQC seeks to educate, empower and improve our client’s quality of life.
The Care Coordinator Specialist (CCS) will assess the needs of the patients at the AIDS Healthcare Foundation (AHF) Health Care Center and enroll patients in case management. CCS provides referrals and links patients to social services and guides the patient towards optimum health outcomes.

This position is a full-time, non-exempt position, and is based in the Brooklyn AHF Health Care Center, near the Atlantic Ave,/Barclays Center train station.
The ideal candidate for the Care Coordination Specialist position will be flexible and detail oriented with strong case management and organizational skills. This person must be comfortable working both as part of a team and independently. This position is based in Manhattan, but may require occasional travel to the main office at Jamaica, Queens.
