AIDS Center of Queens County, Inc. is Hiring a Care Coordination Specialist - Citywide Program Near Brooklyn, NY

The AIDS Center of Queens County, Inc. (ACQC) has a strong commitment to providing HIV/AIDS prevention, care and advocacy for individuals throughout the borough of Queens and beyond. A leading provider of services since 1986 for those who are HIV and at risk, ACQC seeks to educate, empower and improve our client’s quality of life.

The Care Coordinator Specialist (CCS) will assess the needs of the patients at the AIDS Healthcare Foundation (AHF) Health Care Center and enroll patients in case management. CCS provides referrals and links patients to social services and guides the patient towards optimum health outcomes.

Responsibilities for Care Coordination Specialist:

  • Screening patients for services in all AHF clinics in Manhattan, Brooklyn, Jamaica, and the Bronx.
  • Conduct a psycho-social intake assessment for patients with one urgent need and refer to ACQC for other needs.
  • Develop service plans and follow up.
  • Conduct annual reassessments & updates.
  • Work with service providers on the coordination of services.
  • Carry a caseload of 75 clients.
  • Coordinate with the other/departments at ACQC (Mental Health, Prevention, Housing, Legal Services, etc.) to ensure that all patient service needs are met.
  • Assistance with health insurance such as ADAP, Health First etc., link to HRA, homecare and other services as needed.
  • Provide accompaniments as needed.
  • Track follow-ups and outcomes on progress notes.
  • Conduct case closures.
  • Follow up with the Assistant Director on any issues or barriers encountered.
  • Other duties as assigned by the Assistant Director and/or the Director of Case Management.

Education & Required Qualifications Care Coordination Specialist:

  • Bachelor’s degree with one-year of social service experience, or some college coursework with 2 years social service experience.
  • Three to four years demonstrated health-related work experience; experience working with the HIV/AIDS population strongly preferred.
  • Preferably Bilingual (English/Spanish)
  • Must be culturally competent and able to work with individuals from non-traditional backgrounds in terms of gender identity and sexuality. Must be able to convey sensitivity in a non-judgmental manner.

This position is a full-time, non-exempt position, and is based in the Brooklyn AHF Health Care Center, near the Atlantic Ave,/Barclays Center train station.

The ideal candidate for the Care Coordination Specialist position will be flexible and detail oriented with strong case management and organizational skills. This person must be comfortable working both as part of a team and independently. This position is based in Manhattan, but may require occasional travel to the main office at Jamaica, Queens.
