Job Overview:

AHC Home Health and Hospice is looing to hire a dual role RN Case Manager to join our team. As a Case Manager, you will be responsible for coordinating and managing the care of patients in various healthcare settings. This is a vital role that requires strong organizational skills, attention to detail, and the ability to work collaboratively with a multidisciplinary team.

Duties:
- Conduct comprehensive patient assessments to determine their needs and develop individualized care plans
- Collaborate with healthcare providers, including physicians, nurses, and therapists, to ensure the delivery of high-quality patient care
- Coordinate and facilitate communication between patients, families, and healthcare professionals
- Monitor patient progress and make necessary adjustments to care plans as needed
- Advocate for patients' rights and ensure they receive appropriate services and resources
- Provide education and support to patients and their families regarding their healthcare options
- Utilize managed care principles to optimize patient outcomes and minimize healthcare costs
- Utilize utilization management techniques to ensure appropriate utilization of healthcare services
- Collaborate with insurance companies and other payers to obtain authorization for necessary medical treatments or services

Requirements:
- Nursing degree (required)
- Previous experience in case management or a related role (preferred)
- Excellent communication and interpersonal skills
- Ability to work independently as well as part of a team
- Strong organizational skills with the ability to prioritize tasks effectively
