RN Care Coordinator Position Summary

A RN Care Coordinator is responsible for the delivery of care to residents on the unit. Their leadership role includes the following responsibilities: 

·Manage the nursing care for all residents and delegate shift tasks to nurses.

·Participate, observe, and validate the delivery of care to residents while identifying needs, participating in care plans, and administering care.

·Maintain confidentiality of protected health information, including verbal, written and electronic communications.

·Maintain confidentiality of protected health information, including verbal, written and electronic communications.

·Perform other assigned duties as directed by the Director of Nursing or Administrator.

RN Care Coordinator Requirements

·Current unrestricted RN license in practicing state

·Current CPR/BLS Certification
