Position Responsibilities: The Assistant Housing Programs Manager works closely with the Housing Programs Manager to advance the overall goals of AgeSpan's housing initiatives. The position will have both programmatic and supervisory responsibility: three days/week working as a Resident Service Coordinator at a Supportive Housing site, and two days/week providing support, mentoring, and oversight to the Resident Service Coordinator (RSC) team. This position focuses on enhancing individual and team capabilities, ensuring compliance with funding sources, and maintaining seamless coverage at multiple housing sites.
Essential Functions: (Core duties or tasks that are fundamental and not marginal to the performance of the job)
Programmatic:
  • Work on site 20 hours per week/3 days per week
  • Regularly educate residents on the components of the Supportive Housing Program (24/7 emergency response, congregate meals, connection to resources, etc.)
  • Coordinate and facilitate services and activities and maintain scheduled office hours.
  • Conduct surveys at regular intervals to solicit resident feedback on planned programs and activities.
  • Inform residents about State Home Care and other publicly funded programs and public benefits. Make referrals and assist residents with securing benefits as appropriate.
  • Assist in screening residents for Home Care Program eligibility and establish appropriate care plans with AgeSpan Care Manager(s) and Nurse(s) assigned to the sites.
  • Recruit, train and supervise volunteers who assist with group/congregate activities.
  • Identify gaps in services for residents. Develop, implement, and offer a range of services and programs to residents with the goals of establishing and maintaining cost effectiveness and affordable services, and meeting unmet needs.
  • Conduct field visits to assess an older adult's physical, social, emotional, and environmental status to determine their eligibility and needs in their homes in accordance with Executive Office of Elder Affairs regulations.
  • Work closely with community organizations and social service providers to provide programs, supports and services to allow residents to age in place.
  • Provide accurate and timely documentation to meet all contractual and administrative standards as required by the Executive Office of Elder Affairs, AgeSpan, and, if appropriate, the Housing Authority or Property Management. Provide regular updates, feedback, and reports to Housing Authority or Property Management staff to preserve residents' tenancy. Develop annual goals for the site(s) and provide status reports.
  • Organize regular meetings with residents to address social, recreational, and educational needs. Identify problems and issues and approaches to dealing with them.
  • Attend regularly scheduled on-site interdisciplinary meetings with AgeSpan Nurse and Care Manager and vendor representatives to provide on-going monitoring of residents' needs and services.
Supervisory:
  • Offer one-on-one support and in-depth oversight to RSCs during scheduled days, emphasizing day-to-day tasks, bolstering knowledge on programs and available services, and exploring diverse offerings for activities.
  • Observe new hires and provide one-on-one support and feedback regarding day-to-day Resident Service Coordinator tasks.
  • Work in tandem with the Housing Programs Manager to create and maintain a coverage plan, reviewing and ensuring coverage schedules are in place.
  • Assist in building an ongoing database of information needed for those providing coverage at sites.
  • Oversee RSC documentation to ensure accurate and timely documentation to meet all contractual and administrative standards as required by the Executive Office of Elder Affairs, AgeSpan, and, if appropriate, the Housing Authority or Property Management.
  • Coach RSC staff on appropriate updates, feedback, and reports to Housing Authority or Property Management staff to preserve residents' tenancy.
  • Work closely with community organizations and social service providers to provide programs, supports and services to allow residents to age in place. Assist RSCs in building these relationships.
  • Identify ongoing training needs for RSC team, assist in facilitating trainings when able.
  • Provide coverage as a floater at multiple housing sites within the Merrimack Valley and North Shore regions when needed, ensuring continuous support during staff vacancies or absences.
  • On days where floater coverage is not needed, provide support to other RSCs at events and assist community outreach team at scheduled outreach events.
  • Serve as a role model for staff by exhibiting the following: creativity, broader agency participation, motivation, initiative, professional growth, customer service, continuous improvement, collaboration, and flexibility/adaptation.
  • Assist with special projects when not working one on one with new staff.
  • Assist in piloting and brainstorming new initiatives.
Miscellaneous:
  • Attend regularly scheduled RSC team meetings and monthly All Staff and Community Programs meetings.
  • Other related duties as assigned.
Qualifications:
Work history: 1 year(s) experience working as a RSC, current workload is in good standing; has shown reliability and accountability; able to set, as well as follow, limits; demonstrates support for the agency mission; has shown a desire to work collaboratively with management as well as other AgeSpan staff.
BA/BS degree; LSW preferred. Bilingual English/Spanish a plus. Knowledge of housing/disability laws and regulations helpful. Desired leadership qualities: strong organizational, time management, and verbal and written communication skills; positive attitude; ability to delegate as well as take direction; ability to function well in fast-paced, changing environment. Solid computer skills and knowledge of Microsoft Outlook, Excel, and PowerPoint. MARSCH (Massachusetts Association of Resident Service Coordinators) Certification helpful but not required. SHINE (Serving the Health Insurance Needs of Everyone) certification a plus. ServSafe certification a plus. Ability to lift 25 pounds. Must have reliable transportation: position involves travel between multiple housing sites, and flexibility is essential to adapt to the varying environments and unknowns associated with different locations.
