AgeOptions has rewarding opportunities for Social Service professionals with an advanced degree and licensing (M.S.W. or M.A., LSW, LCSW) and L.P.N./R.N. nursing professionals to work in their H.O.P.E. Program. The Home Options Path to Empowerment (H.O.P.E) Program managed through AgeOptions (non-for-profit) helps individuals transition from a long-term care facility and move into their own home in the community. Utilizing interdisciplinary teams of social workers, community care partners, nurses, occupational therapists, social services professionals and outreach practitioners to assist members to transition to community living. Roles are "hybrid" (office, field and remote which can be your home) with work in the field 2-3 days a week (travel in the greater Chicago area and surrounding suburbs).

What is the H.O.P.E. Program?
The Home Options Path to Empowerment (H.O.P.E.) Program, managed through AgeOptions, is funded by the Department of Human Services under the Colbert and Williams Consent decrees is a nursing home transition program. It focuses on helping eligible nursing facility residents transition to community living independently. The program utilizes interdisciplinary teams to provide comprehensive support, life skill building, physical and emotional services and supports, ongoing case management to ensure a safe, successful and person-centered transition for members.

The starting salary range for this role is $44,000-$65,000 annually for social workers and $60,000-$80,000 for Licensed Practical and Registered Nurses; based upon an individual's experience, education and licensing and our internal equity.
