Bilingual Care Manager Position

JOB SUMMARY

This position requires Bilingual-Spanish speaking, reading and writing proficiency. Age Well Care Management team provides care to South Orange County adults over the age of 60 who need assistance and direction in their abilities to live independently at home in a safe environment. This position requires Bilingual-Spanish: speaking, reading, and writing. This position also requires driving to clients’ homes for home visits (mileage reimbursement is provided). To perform this job successfully, an individual must perform each essential duty satisfactorily, communicating effectively with coworkers, clients, families, management and others, and deal efficiently and professionally under pressure. DUTIES

• Assess need for Care Management from community, family, Age Well Senior Services referrals, and provide necessary service coordination as needed.
• Schedule comprehensive in home assessments of clients upon referral
• Assure client and/or appropriate others’ consent to participate in Care Management.
• Develop individualized, written plan of care and services, based on comprehensive assessment of needs and/or resources, mutually with client.
• Arrange for services by referral and/or negotiation with service providers (including informal resources) for the delivery of needed services to client.
• Provide client reassessments every 3 months and Home Visit reassessments annually.
• Submit all statistical data related to the provision of care management and Administrative office.
• Explain and promote community resources to clients, family members, and potential clients who could possibly be eligible for services.
• Able to be flexible and work at multiple senior centers and offices.
• Timely compliance and reporting of monthly/quarterly reports.

REQUIREMENTS

• 2 – 4 years of experience in the field of aging, or equivalent combination of education and experience.
• Bilingual – Spanish (Speaking, reading, writing).
• Knowledge of senior resources available in Orange County.
• Knowledge of human services available to seniors.
• Working knowledge of databases and MS Office.
• Experience working with seniors and older adults.
• High level communication and interpersonal skills.
• Ability to read and interpret documents such as safety rules, procedure manuals and reports.
• Strong organizational, problem-solving, verbal and writing skills.
• Team mindset with strong work ethic.
• Valid California Driver's License, Vehicle Insurance, and Registration

EDUCATION

• AA in Gerontology, Human Services, or Social Services (With Care Management/Case Management experience)
• BSW or BA in Gerontology, Human Services or Social Services (Preferred, not mandatory)
• Registered or licensed in California, if applicable
