LPN On-Call Weekends - Join Our Compassionate Team at Agape Hospice NW

Position Overview:
Agape Hospice NW is seeking a dedicated Licensed Practical Nurse (LPN) for immediate weekend on-call opportunities in the Portland Metro area. As an LPN on weekends, you will play a vital role in providing compassionate care and support to our patients and their families during nights and weekends. While most calls and patient issues are managed by our after-hours phone system and triage system, occasional visits or calls may be required to ensure our patients receive the highest quality of care.

Work Hours:
Friday 5:30pm - Monday 8:30am

Compensation:

• $20 per hour for hourly on-call time, or "beeper rate"
• $46 per hour while providing treatment and during travel to patient's residence (no beeper rate)

Job Responsibilities:

• Provide direct patient care under the supervision of a registered nurse
• Implement current nursing practice following comprehensive assessments and the plan of care
• Document patient services accurately and timely
• Educate patients and families on diagnosis and self-care
• Coordinate hospice services and report needs to the interdisciplinary team
• Utilize equipment and supplies effectively and efficiently
• Manage pain and symptom management for patients and evaluate their response to treatments/medications
• Participate in personal and professional growth and development
• Perform other duties as assigned by the registered nurse

Qualifications:

• At least 1 year of nursing experience; community health/hospice or medical/surgical experience preferred
• Current LPN license in Oregon
• Valid driver's license required
