We are professional, innovative, inclusive and our goal is to Provide care coordination and community supports to individuals with mental illness and behavioral health needs. We are seeking a Youth and Family case manager to serve children with qualifying diagnosis for 1915i funded services.

Responsibilities:

• Use critical thinking skills to assign cases to appropriate caseworkers.
• Maintain documentation for each case.
• Initiate communication between the client and the assigned caseworker.
• Respond to client inquiries and requests.
• Monitor case worker activity; follow-up to ensure that all tasks have been completed.

Job Types: Full-time, Part-time

Pay: $19.00 - $25.00 per hour

Expected hours: No less than 8 per week
