Summit Hospice - Hospice (STNA) - Full Time

Summit Home Care and Hospice is searching for an experienced and motivated Hospice STNA to join our team!

Applicants must hold a valid Ohio STNA license in order to be considered for this position.

What we offer:

Job Type: Full-time

Standard shift: Day shift (8:30a-5:30pm), M-F No weekend or holiday commitments

Salary Range: $18.00-$20.00/hour

Job Location: Based out of our Columbus Office. Territory will be assigned based on patient needs. We provide services to all of Franklin County, and the six surrounding counties.

What the position looks like:

Provides personal care and support to the hospice patient in their home, or nursing facility where the patient resides. Must have an active STNA license


What your responsibilities will be:

  • Assists patients with ADLs such as transfers, bathing, dressing, grooming, and eating.
  • Adheres to the patient's individualized plan of care and notifies the RN Case Manager of changes in patient condition.
  • Assists care givers by demonstrating appropriate ways to lift, turn, and reposition the patient.
  • Delivers supplies to their assigned patients on a regular basis and notifies RN Case Manager/Clinical Manager of any additional supplies needed as patient declines.
  • Records patient information through timely documentation in the electronic medical record.
  • Frequently communicates with the RN Case Manager and/or Hospice Management team of any changes in condition or safety concerns.
  • Maintains a safe, secure, and healthy patient environment by following safety standards and infection control guidelines
  • Updates job knowledge by participating in educational opportunities.
  • Adheres to professional standards, home care policies and procedures, federal, state, and local requirements.
  • Regularly attends Hospice IDG meetings to offer observations related to patients’ changing conditions
  • Active STNA license
  • Previous hospice experience preferred.
  • Must have reliable transportation.

POSITION QUALIFICATIONS

1. Meets the training requirements in accordance with state and federal laws.

2. At least 18 years of age.

3. Ability to read and follow written instructions and document care given.

4. Self-directing with the ability to work with little direct supervision.

5. Empathy for the needs of the ill, injured, frail and the impaired.

6. Possess and maintains current CPR certification.

7. Demonstrates tact, patience and good personal hygiene.

8. Licensed driver with automobile that is insured in accordance with Organization requirements and is in good working order.

Note: Effective August 14, 1990, a person who has successfully completed a state established or other training program that meets the requirements of CFR 484.36(a} and a competency evaluation program, or state licensure program that meets the requirements of CFR 484.36(b), or a competency evaluation program or state licensure program that meets the requirements of S 484.36(b).

JOB LIMITATIONS

The home health aide will not function in any manner viewed as the practice of nursing according to the state's Nurse Practice Act. Specifically, the home health aide will not administer medications, take physician's orders or perform procedures requiring the training, knowledge and skill of a nurse, such as sterile techniques.
