Affinia Healthcare is Hiring a Behavioral Health Consultant - LCSW Near St. Louis, MO

POSITION SUMMARY: The Behavioral Health Consultant (BHC) provides direct behavioral health care in collaboration with medical providers for the purpose of integrating the physical and behavioral healthcare needs of Affinia Healthcare patients. The BHC also serves as a leader for behavioral health integration practices in the primary care setting.

POSITION QUALIFICATION REQUIREMENTS:

Education: Master's degree in social work Required

License(s)/Certification(s) Required: Licensed Clinical Social Worker - LCSW

Experience:

  1. Three years' experience in a health/ mental health setting as a Licensed Clinical Social Worker.
  2. Two years' experience working in an under-served community.

Skills and Abilities which may be representative but not all inclusive of those commonly associated with this position.

  1. Highly skilled in problem-solving.
  2. Familiarity with DSM V diagnostic criteria.
  3. Familiarity with chronic medical conditions.
  4. Excellent written and oral communication skills.
  5. Basic counseling skills.
  6. Familiarity with Motivational Interviewing.
  7. Good computer skills including familiarity with use of an electronic medical record.
  8. Ability to communicate comfortably with medical providers in a fast-paced setting.
  9. Understanding of the effects of structural inequities on health outcomes.
  10. Approaches patient care with an anti-racist and trauma-informed framework
  11. Ability to maintain a pleasant, open, and helpful demeanor.
  12. Maintain a flexible attitude during times of unexpected workflow, while being interrupted.
  13. Ability to distinguish between various roles in the social work profession and operate within the correct role.
  14. Self-motivated, independent worker
  15. Strong time management and organizational skills

Transportation Requirement: Must possess a valid driver's license and automobile with insurance coverage as required by the State of Missouri

FUNCTIONS OF THE JOB:

Essential Functions

  1. Meet with medical team each morning for daily huddle.
  2. Move about the clinic throughout the day to see patients in exam rooms in the integrated, warm hand-off process, in collaboration with medical providers.
  3. Provide behavioral interventions for patients with chronic illnesses to improve their health status.
  4. Provide behavioral assessments and interventions for patients who have positive scores on depression, substance use, and other screening tools used in the health center.
  5. Provide brief assessments that combine knowledge of DSM V criteria, impact of medical conditions, cultural considerations, and systemic barriers.
  6. Provide crisis intervention including risk assessment and appropriate follow - up planning to patients referred by their PCP.
  7. Address social and emotional factors related to the patients' medical conditions.
  8. Collaborate with patient to develop health self-management goals.
  9. Provide feedback to medical provider.
  10. Complete documentation of patient encounters within 48 hours.
  11. Meet monthly productivity expectations.
  12. Perform duties that are required for the PCHH, and other grants as needed.
  13. Coordinate treatment with PCP and psychiatrist as appropriate.
  14. Screen patients for appropriate psychiatry referral.
  15. Provide consultation and serve as a resource to other Affinia Healthcare programs.
  16. Lead or co-lead classes and patient groups as needed to improve patient health status.
  17. Maintain documentation records according to state and agency standards.
  18. Provide scheduled, individual therapy sessions as needed.
  19. Perform other duties as assigned.

Marginal Functions:

  1. Provide educational in-services to the department and organization.
  2. Assist in coordinating special programs such as community events.

Supervises: Students and volunteers as needed
