Aevum Home Health is Hiring a Face to Face Coordinator Near San Luis Obispo, CA

Job Overview:

We are seeking a dedicated and compassionate Medical Assistant to join Aevum Home Health. The Medical Assistant will be responsible for supporting our team of team of clinicians by providing high-quality patient care during visits to patients homes and Assisted Living Facilities. The role includes clinical support, administrative duties, and effective communication with facility staff to ensure seamless patient care management.

Key Responsibilities:

  1. Administrative Duties:
    • Coordinate and schedule patient visits in collaboration with Assisted Living Facility staff.
    • Maintain accurate and up-to-date patient records in electronic health record (EHR) systems.
    • Handle patient check-in and check-out procedures, including verifying insurance information.
    • Assist with billing, coding, and processing insurance claims.
    • Answer phone calls, handle inquiries, and provide information to patients and facility staff.
  2. Patient Care:
    • Provide patient education and instructions on medication, diet, and lifestyle changes.
    • Assist in managing patient referrals and follow-up appointments.
    • Address patient concerns and ensure a positive patient experience.
    • Ensure patient privacy and confidentiality at all times.
  3. Communication and Coordination:
    • Serve as a liaison between the primary care team, home health team and Assisted Living Facility staff or Power of attorney’s for patients.
    • Communicate patient care plans and updates to facility staff to ensure continuity of care.
    • Collaborate with facility staff, home health, hospice, or any other health care providers to address any immediate patient care needs or concerns.
    • Ensure timely communication of test results and provider recommendations to facility staff and patients' families.
  4. Operational Support:
    • Maintain inventory of medical supplies and equipment used during facility visits.
    • Ensure compliance with health and safety regulations.
    • Participate in quality improvement initiatives.
    • Support other clinical and administrative tasks as assigned.

Qualifications:

  • Education: High school diploma or equivalent.
  • Experience: Previous experience in a medical office, clinical setting, or visiting practice is preferred.
  • Skills:
    • Strong organizational and multitasking abilities.
    • Excellent communication and interpersonal skills.
    • Proficiency in using EHR systems and other medical software.
    • Knowledge of medical terminology and basic clinical procedures.
    • Ability to work in a fast-paced environment and handle stressful situations.
    • Ability to adapt to different Assisted Living Facility environments.

Physical Requirements:

  • Ability to stand, walk, and sit for extended periods.
  • Ability to lift and move medical equipment and supplies.
  • Manual dexterity to perform clinical tasks.
  • Visual and auditory acuity for patient assessment and care.
