Job Summary: The Registered Nurse in the Acute Care setting manages the patient/family experience through utilization of critical thinking, teamwork, effective communication, and nursing judgment/knowledge/skills. Provides indirect and direct care to patients ranging in age/developmental stages from infants (pediatrics) to geriatrics. Uses the nursing process of assessment, planning, implementation and evaluation to meet the physiological, psychosocial, safety, learning and self-care needs of patients and their families. Responsible for managing care of assigned patients and directing activities of ancillary staff. Provides individualized, goal directed nursing care to patients through the use of the nursing processes & documentation from admission to discharge. This includes & is not limited to patient assessment by gathering information necessary to formulate a nursing diagnosis to be used as a basis for planning care consistent with formulated nursing diagnosis. Implements care that is skillful, efficient, safe & based on patient care standards by performing nursing procedure required for patient assignments, documenting the care given & changes in the patient's condition in accordance with standards. Anticipates & recognizes potential problems & intervenes appropriately by correlating data, test results, progress notes & patient condition. Evaluates the degree of effectiveness & makes changes as needs are recognized. Provides leadership by managing care through others using Standards of Practice (JCAHO, Title XXII, Professional Practice Acts) & interpreting (or uses others to interpret) Standards of Practice to other staff which may include legal implications. Actively participates in professional growth opportunities & provides high quality customer relations. Performs other duties as assigned. Principal accountabilities: Triage Follows triage policy - screens patients within 5 minutes of presentation, reassesses patients within guidelines of triage policy and documents reassessment. Performs an initial physiological, psychological and social assessment. Completes all documentation and appropriately assigned triage level. Initiates standing orders as appropriate. Utilizes customer service standards in all interactions with patients and families. Reassesses patients in waiting area according to triage category and documents results Communicates with patients and families waiting in lobby about delays and timeframes. Critical Care Documentation accurately reflects application of nursing process - triage assessment, pain assessment, discharge instructions, etc. Demonstrates the ability to obtain information and interpret information regarding the patient as evidenced by assessments, documentation and interpretation of findings. Reports significant findings of initial assessment to appropriate health care personnel in a timely manner and charting reflect notification. Completes nursing documentation forms by the end of each shift. Individual physician orders are signed off immediately upon completion - time, date and signature. Identified educational needs of patients and families. Provides appropriate education - discharge teaching sheets, discharge instructions. Organizes patient care and demonstrates ability to set priorities, completing assignments in an efficient, outcome oriented manner. Schedule Notes:13 weeks - night shiftSkills: Experience/Knowledge/Skills: Independently practices and prioritizes multiple demands in an ever-changing environment. Good interpersonal, communication (both oral and written) and teamwork skills are required. Special Requirements: Current Licensure California RN or Interim Permitee; BLS - Health Care Provider, ACLS, PALS, TNCC. Must have demonstrated knowledge of basic nursing skills and effective English written and communication skills. Education/Certification/Licensure: RN, BLS, ACLS, PALS- Required TNCC, BA/ BS - Desired Communications/Customer Relations Interacts in a professional manner with co-workers, other departmental staff and physicians to promote a positive atmosphere in the unit and hospital. Utilizes effective communication skills, both verbal and written, in interactions with all team members. Utilizes calm, direct, professional manner in dealing with difficult situations, recognizing and utilizing the appropriate chain of command and reporting mechanisms (PIRs). Maintains confidentiality of patient data, familiar with and educates others on HIPPA requirements, keeping discussion out of public areas. Interacts with patients and families with sensitivity and understanding. Seeks out physician/team members for professional consolation and collaboration to discuss patient care goals. Addresses emotional, physical, and spiritual needs of patients and family. Demonstrates ability to provide and accept constructive feedback, remains approachable, listens to others and offers appropriate suggestions. Demonstrates the ability to efficiently and effectively delegate in a professional supportive manner. Performs routine charge nurse rounds. Assesses patient care needs. Proactively identifies potential areas for intervention. Communicates utilizing techniques of empathy, fairness and honesty. Values others input, is supportive and reinforces positive behaviors. Planning/Leadership/Teamwork: Utilizes leadership concepts and skills in care delivery prioritizing care in an efficient, goal directed manner. Provides clinical supervision and delegates' appropriate tasks to LVN, CNA, unit secretaries, admissions clerks and student nurses. Utilizes resources for decision making, including peers, supervisors, ancillary departments, physicians manuals and other literature, recognizing all options available and identifying appropriate resolutions. Actively supports the departments' philosophy and standards of practice. Demonstrates knowledge of patient care guidelines and policy as demonstrated through care delivery and documentation. Maintains a calm demeanor conductive to effective problem solving and smooth unit operation. Actions demonstrate a team/multidisciplinary approach to patient care. Demonstrates flexibility in hours and shifts to meet the needs of the department Monitors and expedites patient flow through the department. Professional Growth and Development Accepts responsibility for own actions, and takes a proactive approach to problem resolution. Maintains and updates own knowledge skills, attending in-services/staff meetings, seminars, etc. Seeks learning opportunities within the department to expand/challenge knowledge base and skills. Participates in orientation and precepting of new employees.
