Job description

Aequor, LLC is now hiring a Physical Therapist in Stockton, CA for a 13 week contract (with possibilty to extend).

Responsibilities:

• Conduct comprehensive assessments of clients' physical health, including mobility, strength, balance, and functional abilities.
• Develop individualized treatment plans based on assessment findings and clients' goals.
• Provide hands-on physical therapy interventions to improve range of motion, strength, coordination, and flexibility.
• Educate clients and their caregivers on therapeutic exercises, proper body mechanics, and injury prevention strategies.
• Monitor clients' progress and adjust treatment plans as needed to achieve optimal outcomes.
• Communicate effectively with interdisciplinary team members, including physicians, nurses, and other therapists, to coordinate care and ensure continuity of services.
• Document all evaluations, treatments, and progress notes accurately and timely in accordance with regulatory requirements and company policies.

Qualifications:

• Baccalaureate degree in Physical Therapy.
• Current license from the California Board of Medical Licensure and Supervision as a Physical Therapist.
• Minimum of one year of experience in a home health setting
• Strong clinical skills with expertise in orthopedic and neurological rehabilitation.
• Excellent interpersonal and communication skills with the ability to establish rapport with clients and their families.
• Demonstrated ability to work independently and as part of a multidisciplinary team.
• Valid driver's license and reliable transportation.
