AEIC Case Management LLC is Hiring a Community Services Specialist-I Near North Richland Hills, TX

Job Summary: We are seeking a dedicated Community Services Specialist to join our team for the C.H.A.M.P. & P.A.T.H.S. program at AEIC. As a Community Services Specialist, you will be responsible for providing comprehensive case management services, skills and development training to children and adults in the Dallas/Fort Worth areas, as well as ensuring they receive access to necessary healthcare, social services, and support. This is a contract position with the opportunity to make a meaningful impact on the lives of vulnerable populations.

Responsibilities:

  1. Conduct initial assessments (CANS/ANSA) to determine the level of care for children and adults to be admitted to either the C.H.A.M.P. or P.A.T.H.S. program.
  2. Implement/provide skills and development training based on individualized care plans in collaboration with clients/parents, to address outlining goals, objectives, and interventions.
  3. Coordinate access to healthcare services, including medical, dental, vocational, educational, and social for clients.
  4. Advocate for clients and ensure they receive timely and appropriate services from community resources, including but not limited to housing, transportation, food assistance, and childcare.
  5. Provide the following services as needed: routine/family/intensive case management, family training, and medication training/support, in addition to skills and development training.
  6. Monitor client progress and reassess needs regularly to request adjustments to care plans as necessary to promote positive outcomes. 
  7. Maintain accurate and up-to-date documentation of client interactions by ensuring 95% of notes are completed within 24 hours of the date of service for review, assessments, care plans, and progress notes in accordance with program requirements. (Must maintain professional verbiage, grammar, and punctuation)
  8. Collaborate with interdisciplinary teams, including healthcare providers, social workers, educators, and community organizations, to ensure coordinated and holistic care for clients.
  9. Participate in program meetings, trainings, and quality improvement initiatives as required.
  10. Adhere to all program policies, procedures, and ethical standards, maintaining client confidentiality and privacy at all times.
  11. In-person meetings for 7 to 9 clients per week with a minimum of 28 hours per week.

Qualifications:

  • A high school diploma or a high school equivalency certificate issued in accordance with the law of the issuing state (required)
  • At least three years of continuous documented full-time experience in mental health rehabilitative services or case management services experience in case management. (required)
  • Must complete standardized training
  • Knowledge of Texas healthcare and social service systems, including Medicaid and CHIP, is preferred.
  • Strong interpersonal skills with the ability to build rapport and establish trust with clients from diverse backgrounds.
  • Excellent communication skills, both verbal and written, with the ability to effectively advocate for clients and collaborate with interdisciplinary teams.
  • Ability to work independently, prioritize tasks, and manage time effectively in a fast-paced environment.
  • Proficiency in computer skills, including Microsoft Office and electronic health record systems.
  • Bilingual proficiency in English and Spanish is preferred. (Incentive if Bilingual)
  • Must undergo a background check, TB Skin Test, Drug Testing, and CPR/First Aid training.
  • Valid driver's license, current auto insurance, and reliable transportation. (required)

Schedule:

1. Self-Pace specialized hours 

2. Weekends as needed 

This is a contract position meaning 1099, with a varying contract length. Full-time, part-time and PRN positions are available. AEIC Case Management LLC does not offer mileage reimbursement and will only pay hours billed as an independent contractor for hours spent with the client directly or telehealth due to the recent COVID-19 pandemic. Applicants will be required to submit all orientation paperwork and complete the training process before being assigned to a client.
