Advocate Aurora is Hiring a Remote LPN Aurora At Home Weekend Program

Major Responsibilities:
  • Performs basic assessments, collects data, identifies problems and follows a care plan developed by the Registered Nurse.
  • Administers medication and treatments. Provides wound care, hot/cold compresses, patches, G-tube, obtains specimens, takes patients vitals, and performs other medical procedures as documented in the plan of care.
  • Communicates with Registered Nurse on the status of the patient after each visit and any changes in their patient's condition. Communicates any problems and documents the plan and actions taken in the patient's visit report.
  • Follows up with the patient's physician as needed, including taking verbal orders.
  • Participates in chart audits, Continuous Quality Improvement activities and in services as required. Records observations completely, accurately, legibly and concisely.
  • Documents all information regarding the patients. Maintains and updates accurate clinical and patient records according to agency, state and federal guidelines.
  • Communicates effectively with patient, family, or significant others and provides them with necessary emotional support.
  • Performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.
  • Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
Licensure, Registration, and/or Certification Required:
  • Licensed Practical Nurse (LPN) license issued by the state in which the team member practices, and
  • Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required, and
  • A valid drivers license issued by the Division of Motor Vehicles, or
  • Driver license issued by the state of Illinois.
Education Required:
  • Advanced training beyond High School that includes the completion of an accredited or approved program in Practical Nursing.
Experience Required:
  • Typically requires 1 year of experience in performing assessments and providing treatments.
Knowledge, Skills & Abilities Required:
  • Demonstrated knowledge and skills to collect data relative to the patient's status, and interpret and report the information.
  • Ability to interact well with other team members, patients, physicians and family members of the patient.
  • Excellent communication skills. Strong interpersonal skills and the ability to work with a variety of clients with patience and respect.
  • Basic computer skills including the ability to document and access information in an electronic system.
  • Must have a tolerance for differences and an appreciation of multi-culturalism and diversity of the patients and their families.
  • Must possess and provide proof of a valid Driver License and a registered, functional car with adequate insurance coverage.
Physical Requirements and Working Conditions:
  • Sit, stand, walk and climb stairs for a limited to prolonged period of time including bending, kneeling, squatting and occasionally reaching one or both arms overhead.
  • Must have the physical ability to move about in confined spaces, including bending, twisting, kneeling, squatting and occasionally reaching one, or both arms overhead.
  • Must be able to:
  • Lift up to 50 lbs from floor to waist.
  • Lift up to 20 lbs from waist to overhead.
  • Lift and carry up to 40 lbs. at waist height reasonable distance.
  • Must be able to:
  • Push/pull with 30 lbs. of force.
  • Perform a sliding transfer of 150 lbs. with a second person present.
  • Subject to exposure to chemical and hazardous waste and communicable diseases.
  • Must have accurate vision to read gauges and measuring devices and to interpret written instructions.
  • Subject to extreme temperature changes due to weather conditions. Occasionally subject to hazardous conditions due to weather, disrepair of patient property, domestic animals and daily travel.
  • Must be able to work safely with people of various sizes and weight with diverse and complex medical conditions and physical limitations.
  • Operates all equipment necessary to perform the job.
