Job Summary:

Performs surveillance, analysis, interpretation and reporting of Healthcare Associated Infections (SNF) and infection prevention education. Writes and updates infection prevention policies and procedures to ensure compliance with regulatory standards of the Accrediting Organization(s); such as The Joint Commission (TJC), Occupational Safety and Health Administration (OSHA), Centers for Medicaid/Medicare Services (CMS), Centers for Disease Control and Prevention (CDC) and other national and state agencies' recommendations and requirements. Is proficient in long-term care epidemiology principles and is responsible for the surveillance, analysis, interpretation and reporting of healthcare associated infections.

Job Requirements:

Education and Work Experience:

  • Associate's/Technical Degree or equivalent combination of education/related experience: Required
  • Bachelor's Degree in nursing (BSN) or equivalent combination of education/related experience: Preferred
  • Two years' experience in long term and/or acute care setting: Required

Licenses/Certifications:

  • Professional training as a licensed nurse, medical technologist, microbiologist, epidemiologist, public health professional, or training in other health care related field: Required
  • Must be qualified by education, training, clinical or health care experience, or certification, and must have completed specialized training in infection prevention and control: Required
  • Certification in Infection Control and Epidemiology (CIC) Certification: Required within 3 years after hire date
  • Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification: Preferred

Facility Specific License/Certifications:

  • Hospital Fire and Life Safety (HLFS): Required

Essential Functions:

  • Conducts surveillance and epidemiology reviews for implementation of the surveillance plan for services provided and populations served utilizing National HealthCare Safety Network standard definitions to classify events, outcomes, and reports. Provides reports to appropriate customers and key stakeholders for significant findings and trends using epidemiologic principles with analysis to drive change and improvement. Creates the infection prevention quality control plan (IPCP). Be an active member of the facility's quality assessment and assurance committee and report to the committee on the IPCP on a regular basis.
  • Integrates surveillance with improvement plans, existing regulations, national and state standards and guidelines. Utilizes evidence-based strategies to influence practice. Conducts rounds to monitor infection control practices of the residents, healthcare workers, volunteers, and other department staff â€" and provide guidance and teaching as appropriate. Conducts rounds to verify the environment sanitation status and procedures, report findings and provide guidance and teaching as appropriate. Ensures the appropriate personal protective equipment is available for the healthcare workers through a partnership with materials management team and is used appropriately.
  • Shares knowledge and skills with others to highlight the importance of evidence-based strategies while bringing creativity and innovation to practice engaging with the adult learner. Works collaboratively with multiple levels of the organization including physicians and executives to design prevention strategies using performance improvement methods while aware of clinical and financial implications.
  • Review and analyze infectious disease laboratory reports and consult with the Director of Nursing Services. Maintains a knowledge base of current Infection Prevention and Control information through networking, literature review and professional meetings. Provides infection prevention and control training to healthcare workers and supports educational programs to promote infection prevention measures across the system. Shares published research findings to support and promote best practice.
  • Partners with local, state and federal regulatory bodies to ensure surveillance, outbreaks and regulations are met. Reports communicable disease data, including disease death related, as required by local, state and federal regulatory agencies. Works collaboratively with leadership in the notification process of communicable disease cases to the regulatory agencies, residents, and their representatives.
  • Performs other job-related duties as assigned.
