• PHYSICAL REQUIREMENTS (b): This job requires frequent bending, squatting, kneeling, climbing, reaching above shoulders, sitting, walking inclines and declines, standing, talking, hearing, and performing repetitive hand motions. Vision requirements include close vision and the ability to adjust focus. The employee will occasionally pull, lift, transfer, or push a patient side to side, to a sitting position, to a standing position, or laterally. The employee must demonstrate a pull lift of 50 pounds from the floor.
• POTENTIAL EXPOSURE TO BLOOD & BODY FLUIDS: Category 3 for potential exposure to blood/body fluids. (Does not require the performance of procedures or other tasks in the work routine that involve exposure to blood, body fluids or tissues, and the assisting in cases of emergency medical care or first aid is not a requirement of employment. Tasks that involve handling of implements or utensils, use of the public or shared bathroom facilities, telephones, and personal contacts such as handshaking, are all considered Category 3 tasks.)

Job Roles

Fitness Health Coach (Outpatient) (AHCL)

• Assesses patient pain interfering with optimal level of function or participation in rehabilitation; makes appropriate physician contact of intervention.
• Coordinates with members of the integrated chronic care team to implement patient care plans.
• Documents client activities and coaching/counseling sessions in established clinical format.
• Ensures completion of orientation paperwork for new office staff. Completes occurrence reports per hospital policy and procedure.
• Implements patient education care plan based upon identified patient needs and evaluates effectiveness of learning. Includes family in teaching as appropriate.
• Interacts professionally with patient and family and involves patient family in the implementation of written plan of care developed by the LCSW and/or Physician staff. Develops specific, measurable, realistic plans and objectives.
• Maintains and meets expectations on time for all competencies, license, certifications and education requirements as outlined by local administration, Adventist Health (AH), The Joint Commission (TJC), Centers for Medicare and Medicaid Services (CMS), and all other regulatory agencies.
• Participates in case conferences to discuss integrated chronic care team responsibilities, patient progress, plans for continued care, new problems, etc.
• Performs components of an appropriate assessment on all patients as related to the therapy requested and provided.
• Promotes wellness and provides necessary coaching, encouraging adoption of habits that are conducive to a higher quality of life.
• Provides coaching, counseling, consultation, information and referral services to patients managing various chronic health conditions.
• Provides treatment as directed by the nurse practitioner and/or physician. Observes and evaluates treatment effect, discusses observations with nurse practitioner and/or physician.
• Responsible for supporting operational aspects of the program to meet patient needs and satisfaction.
• Reviews and documents pertinent medical history, current diagnosis, and pharmaceutical data.
• Willingly performs other duties and innovations as assigned.
