Location: 2325 S DOWNING ST, Denver, 80210
The role you'll contribute:
Coordinates all processes, support, communication, and documentation involved in transferring patients from one facility to another AdventHealth facility, as well as support physician consults. Transfer Coordinators also required to recognize and uphold all regulatory requirements, while simultaneously updating key stakeholders of timelines. Accurately enters all required data into EPIC database. Coordinates with Flight for Life for patient transports.
The value you'll bring to the team:
  • Coordinates requests for transfer of patients into the AdventHealth system, or requests for medical consultation, including connecting the referring MD with the accepting/consulting MD.
  • Coordinates with receiving facility bed availability for the patient and offering Flight for Life transport coordination if applicable.
  • Facilitate all communications between referring and receiving physicians and other hospital staff, while relaying information accurately.
  • Accurately document and enter all required information into the EPIC database as it occurs, for creation of real time data collection.
  • Professionally and accurately represent all AdventHealth facies. Ability to develop and maintain referral relationships with outlying facilities through consistent exceptional customer service.
  • Supports referral patterns in collaboration with Program Director and Medical Director. May be asked to suggest a receiving site based on clinical expertise of a patient condition.
  • Has a strong understanding of the geography of our acute care facilities. Understands the transportation requirements for patients referred from our rural affiliates and sites that frequently refer patients to our facilities.
  • Ability to multi-task while utilizing Windows-based console Dispatcher software and system for multiple incoming calls, patching with receiving physicians and facilities, etc.
  • Supports Outreach Services, Trauma Services and other departments and physician groups in provision of data and coordination of follow up.
  • Adapts to rapidly changing technologies, policies, regulations, and staffing through priority setting and flexibility.
Qualifications

The expertise and experiences you’ll need to succeed:
Required:
  • High school diploma or GED
  • One to two years of direct clinical experience in Emergency Department/Critical Care Unit (such as Critical Care Tech EMT-B etc.), or one to two years of full time direct administrative healthcare experience (such as Nursing Unit Secretary or Patient Access Representative) or one to two years Transfer Center experience.
  • Basic Life Support Certificate (American Heart Association)
Preferred:
  • Bachelor's degree
