The community you’ll be caring for:

  • The Maitland Office Plaza houses our highly skilled teams that support our hospital system including Marketing, Patient Financial Services, Revenue Management, the Credit Union and Human Resources.

The role you’ll contribute:

The Program Manager of Care Transformation is a multi-faceted role that reports to the Director of Care Transformation and collaborates with the Medical Director/s of Quality and Utilization. The program manager will support leadership that will serve the needs of the Integrated Health Services (IHS) and the Clinically Integrated Network (CIN) in managing programs designed to transform the care delivered across the continuum, through care pathways and other innovations. Responsible for driving outcomes and achieving goals aimed at diminishing unnecessary clinical variation, promoting evidence or consensus based best practices that drive improvements in quality, cost of care and consumer experience.

The value you’ll bring to the team:

  • Works with and helps coordinate efforts in the Central Florida Division (CFD) for the deployment of care pathways that are safe, effective and timely.
  • Works with leaders across the continuum of care to develop ambulatory clinical care pathways for physician members of the CIN.
  • Partners with the Director of Care Transformation and the medical director/s of quality and utilization to monitor pathway and monthly performance metrics across the division. Develops reports for senior leaders reflecting program performance successes and opportunities. Works with team to develop and implement performance improvement plan.
  • Works with regional System Care Reliability councils to ensure care transformation alignment across CFD.
  • Deploys program dashboards to manage adherence to and outcomes of clinical pathways in conjunction with physician leadership and analytic resources.
  • Liaison between CFD-North and CFD-South and CIN business units to ensure maximum alignment of transformation strategies and goals.
  • Prepares, analyzes and presents reports and recommendations for leadership regarding program, services and other applicable areas of interest to provide concise and accurate information that aids in decision-making.
Qualifications
The expertise and experiences you'll need to succeed:

EDUCATION AND EXPERIENCE REQUIRED:

  • Bachelor’s degree in Nursing.
  • Three years’ experience across acute care or PAC settings.
  • One-year of Process Improvement or analytic experience.
  • One-year of management/supervisory experience.
  • Current valid State of Florida or multi state license as a Registered Nurse
