Schedule: PRN- As Needed

Shift: Day Shift

Location: Home Health visits in Jackson or Clay County.

The role you’ll contribute:
This position is accountable for assisting the health professional by providing personal health care services and assistance with activities of daily living to the patient as specifically identified in a written assignment and necessary for the accomplishment of patient care plan goals.
The value you’ll bring to the team:
  • Providing Home Health patients with assist in activities of daily living such as bathing and personal care.
  • Providing Home Health patients with assist in instrumental activities of daily living such as preparing meals, shopping for groceries or personal items, performing light or heavy housework, doing laundry, and using a telephone.
Qualifications

The expertise and experiences you’ll need to succeed:

EDUCATION AND EXPERIENCE REQUIRED:

  • Diploma from an accredited school/college of nursing required.
  • BSN Degree within 3 years of employment.
  • Minimum of 1-year Registered Nurse experience in Hospice and/or Acute Care Nursing experience.
EDUCATION AND EXPERIENCE PREFERRED:
  • Bachelor of Science in Nursing.
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
  • BLS Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or job transfer date.
  • Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date.
  • Maintain a current, valid Illinois Driver's License in good standing with a safe driving record and a reliable vehicle.
  • Maintain valid personal automobile insurance.
LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED:
  • Certified Hospice and Palliative Nursing certification
