Schedule: Full-time Shift: Day

Location: 7700 S BROADWAY, Littleton, 80122

The role you'll contribute:

Provides overall coordination for care management in the acute care continuum by establishing a comprehensive program which includes case management with utilization review, care coordination, discharge planning, and social work. Supervises and coordinates the delivery of all Case Management Services including acute care, rehabilitation, and outpatient case management services. This includes staff education, hiring, coaching, performance management and mentoring of staff. Responsible for financial management of department including review of productivity and staffing. Responsible for operations of CMS department including coordination of UM committee and UM reports. Serves as a clinical resource person for case management services for other hospital departments. Acts as a community liaison with external review agencies, home care providers, durable medical equipment providers, medical staff and other community resources used for discharge planning. Has oversight of Key Performance Indicators for Case Management and provides feedback and direction to Case Management team. Targets are met so that case management remains quality focused.

  • Supports and interacts closely with CM team to facilitate positive “team” environment.
  • Thorough understanding of legal, regulatory, and compliance (HIPPA, EMTALA, Joint Commission, Centers of Medicare, and Medicaid) impacting the care delivery and reimbursement process.
  • Expert level of understanding of the application of clinical criteria screening with knowledge of all insurance requirements for patient status based on disease processes and treatment requirements (supports RN in the Director role)
  • Conduct regulatory audits for appropriate patient status, regulatory notifications, and discharge planning requirements for Conditions of Participation with Medicare.
  • Responsible for daily review of staffing and productivity.
  • Meet with Quality Management Director to discuss Case Management impact on quality metrics and create direction for department.
Qualifications


The expertise and experiences you’ll need to succeed
:

Required:

  • High School Diploma OR GED
  • Current Registered Nurse License Colorado.
  • 5 years of experience in Nursing, 3 of which include Case Management in a hospital setting.
  • Prior supervisory/management experience in Case Management.

Preferred:

  • Bachelor's degree: Nursing
  • Experience in Acute Care setting, including discharge planning and clinical utilization review.
