The community you’ll be caring for: Adventhealth Daytona Beach

The role you’ll contribute: Responsible for continuity of care across the continuum with emphasis on quality patient outcomes and cost-effectiveness through decreased length of stay, and control of resource utilization. Provides Patient/family instruction, scheduled physician appointments, any additional discharge collaboration/coordination in the hospital's EMR and coordinates activities according to patient's need.

• Follows established criteria for utilization of services to ensure denials are avoided

• Utilizes nursing data base to initiate discharge planning by procuring appropriate level of care at discharge, i.e. self[1]care, nursing facility, home health, physical / psych facilities

• Acts as liaison with physician to assure appropriate and complete documentation Prioritizes activities and completes work assignments on a timely basis while functioning with minimal supervision

• Assists physicians by providing information, suggestions, alternatives, and timely referrals for discharge planning

• Utilizes nursing data base to initiate discharge planning by procuring appropriate level of care at discharge, i.e. self[1]care, nursing facility, home health, physical / psych facilities

Reviews patient medical records for PI / Risk Management per established criteria and provides appropriate documentation of adverse patient occurrences

The expertise and experiences you’ll need to succeed:

Minimum qualifications:

• High School Grad or Equivalent

• Three years LPN health care experience

Preferred qualifications:

• 5 years' clinical experience

• Licensed Practical Nurse /Vocational Nurse
