Advanced Primary Care Practice Llc is Hiring a Telehealth Liaison, LVN - Advanced Primary Care Practice Near Addison, TX

LVN Required. Remote/Field position. Afterhours 2pm or later start time and/or weekends

Job Summary:

The Telehealth Liaison is responsible for triaging SNF patients that are appropriate for Telehealth visits. This will include taking vitals and initiating Telehealth call to Advanced Primary Care Practitioner via Teams communication. Liaison will be responsible for tracking daily encounters and monthly overview of remote productivity.

Qualifications:

  • LVN required
  • Must have at least one-year experience in the Healthcare industry
  • Knowledge of Long-Term Care is preferred
  • Must possess a valid state driver’s license
  • Must have excellent communication skills; ability to interact well with a great diversity of individuals
  • Self-starter with the ability to prioritize multiple tasks & deadlines with minimal supervision
  • Must have a good understanding of the Federal, State, and local laws/regulatory guidelines governing the operation of a SNF.
  • Goal driven, motivation for growth, territory management, strong presentation skills, performance management, building relationships, emphasizing excellence and results driven
  • Ability to work with outside and inside clients and vendors, and ability to interact positively with all levels of personnel across all functions
  • Should be able to utilize reports and trends to support efforts and to understand clinical status and progress as it pertains to patient care
  • Advanced computer skills including Microsoft Office and aptitude to learn company specific systems
  • Must be able to maintain confidentiality regarding patient, employee, and company proprietary information
  • Must have the ability to relate professionally and positively and work cooperatively with patients, families, and other employees at all levels

Essential Functions:

  • Review admission records, meet with the patient/families and ensure that consent to treat form is completed for Telehealth.
  • Initiate in-person Telehealth visit via Microsoft Teams on behalf of/to treating provider.
  • Communicate and coordinate facility Telehealth schedule for the week between all parties.
  • Responsible for all additional Telehealth duties.
  • Local travel (Houston) required.
  • Respond to facility and patient needs in a timely and professional manner
  • Communicates with senior, regional, and local staff as needed regarding focus
  • Resolve administrative and operational issues as needed
  • Demonstrate commitment, professional growth and competency
  • Promote company philosophy and administrative policies to ensure quality of care
  • Develops and maintains relationships with SNF to review daily admissions and efficiently meet the QIPP requirements and daily/weekly needs of the community at large.
  • Maintains a close working relationship with internal key stakeholders to assure appropriate and efficient care transitions.
  • Deliver a high level of customer satisfaction - Monitor and evaluate customer satisfaction.
  • Responsible for meeting and/or exceeding performance goals.
  • Assist with all other activities and duties as assigned.
