We are looking for a PRN (as needed) Registered Nurse to add to our Home Health Agency in the Anthem area! Advanced Home Health and Hospice of Arizona provides services within patients’ homes valley wide. Looking for a Registered Nurse with some home health experience to provide excellent care to our patients.

Qualifications:

• Graduate of an approved school of professional nursing who is licensed as a Registered Nurse by the state in which practicing.
• Current license as a Registered Nurse in the state or practice. Baccalaureate degree preferred.
• Ability to supervise and direct members of client care team as well as demonstrate organizational and time management skills.
• Demonstrated strong and effective written and verbal communication skills and good interpersonal skills and the ability to deal tactfully with clients and the community.
• Licensed driver with automobile insured in accordance with accordance with state and/or Agency requirements and in good repair.

ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY

• Nursing Services are provided by a qualified RN in accordance with: the state’s Nurse Practice Act; facility policies and procedures; Medicare guidelines.
• Completes comprehensive assessments including OASIS data set within forty-eight (48) hours of referral for unless other arrangements are made.
• Considers the physical, psychological, and socioeconomic needs of the client, the level of family acceptance, and the ability for client needs to be met in the home.
• Works with a team of RNs, LPNs, Therapists, Social Services, and Home Health Aides to provide effective and quality home care services.
• Demonstrates knowledge of safety infection control practices by compliance with policies and procedures.

Pay: $45.00 - $120.00 per hour
