We are looking for a Registered Nurse to join our Home Health team. The ideal candidate will have experience in providing care to patients in their own homes and be able to assess, plan, implement, and evaluate patient care plans. The Registered Nurse will also be responsible for providing education and support to patients and families, as well as collaborating with other healthcare professionals to ensure the best possible outcomes for patients. The successful candidate will have excellent communication skills and be able to work independently with minimal supervision.
