Position Summary

Aspen Ridge Home Health and Hospice is looking for an outstanding Home Health Wound Care Nurse to add to our home health team. Demand for wound care is increasing for our company and we would like to increase efficiency and improve outcomes for our patients and our collaborative wound care providers. Many of these wounds are chronic in nature (pressure related injuries, venous stasis ulcers, diabetic ulcers, etc.) and as such require very specific wound care orders to be input into our EMR and then followed precisely. We need a self-motivated wound care nurse who enjoys making home visits.

Responsibilities

• Maintain and improve established wound care provider relationships
• Reach out to providers for clarification
• Daily notes and visits in MatrixCare completed in 24 hours
• Wound care order and care plan intervention writing in MatrixCare
• Medicare Oasis recertifications
• Travel between patients for in home visits and manage your own schedule

Coverage Area:

This position would cover Davis, Salt Lake and Weber counties and be available Monday through Friday 9am – 5pm.

This opening is full-time salaried position with benefits available and mileage reimbursement for travel. (Mileage is reimbursed at $0.65 per mile.)

Requirements:

• Current Utah RN license
• BLS certification
• Previous wound care experience
• Current UT Driver’s license
• Wound care specific certification preferred but not required

Job Type: Full-time

Pay: $36.00 - $39.66 per hour
