Advanced Care Home Health is seeking a compassionate and highly motivated Registered Nurse Case Manager to join our dedicated team. As a Registered Nurse Case Manager, you will play a crucial role in providing exceptional care and support to our patients in the comfort of their own homes. Responsibilities:

  • Conduct comprehensive assessments of patients' health status and individualized needs to develop personalized care plans.
  • Coordinate and manage patient care in collaboration with physicians, therapists, and other healthcare professionals.
  • Provide skilled nursing care, including medication administration, wound care, and monitoring of vital signs.
  • Monitor patients' progress and update care plans accordingly, ensuring that interventions align with their evolving needs.
  • Educate patients and their families on disease management, self-care techniques, and available community resources.
  • Advocate for patients' rights and ensure their preferences and goals are integrated into the care plan.
  • Maintain accurate and up-to-date documentation of patient assessments, care provided, and outcomes achieved.

  • Adhere to regulatory guidelines, company policies, and industry best practices.

Qualifications:

  • Valid Registered Nurse (RN) license in Oregon.
  • Minimum 2 years of nursing experience, preferably in home health or a related field.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills to establish rapport with patients, families, and healthcare professionals.
  • Ability to prioritize and manage multiple cases efficiently.
  • Demonstrated compassion, empathy, and sensitivity in providing patient-centered care.
  • Knowledge of Medicare guidelines and home health regulations is desirable.
  • Proficiency in electronic health record (EHR) systems and basic computer skills.
