Job Summary:
We are seeking compassionate and dedicated Direct Support Professionals to provide care and support to individuals with developmental disabilities. As a Direct Support Professional, you will play a vital role in enhancing the quality of life for those in your care. This position requires knowledge of HIPAA regulations, experience in patient care, and a strong understanding of behavior management techniques.
Responsibilities:
- Provide direct care and support to individuals with developmental disabilities
- Assist with daily living activities such as bathing, dressing, and meal preparation
- Monitor and record vital signs as required
- Implement behavior management strategies to promote positive behaviors
- Support individuals in participating in social and recreational activities
- Collaborate with healthcare professionals, including nurses and social workers, to ensure comprehensive care
- Maintain accurate and up-to-date documentation of services provided
· Complete individual assessments and evaluations to determine the interests, strengths and needs of the individual if applicable.
· Plan and coordinate and /or provide travel training and /or transportation for individuals (including the use of personal car) as necessary.
· Complete individual-based assessments and evaluations to determine the interests, strengths and needs of the individual if applicable.
· Enable an individual to attend community events.
· Assist an individual participating in activities such as: assistance in completing activities of daily living, ordering off a menu, purchasing items, learning basic cooking, laundry skills, etiquette, travel training, accessing activities in the community and /or others as per service plan. Provide regular feedback to family/ guardian in regard to individual’s progress according to their needs and concerns. Request feedback from families/ guardians and act on these concerns.
· Record and report all billable hours for Medicaid and any other funding source.
· Ability to work flexible schedule based on consumer eyed/ preference including potential nights/ weekends.
· Complete all necessary Placement logs, Activity reports, Intervention Plans
· Act as an ambassador for our agency in all community settings.
· Perform additional duties as required by the nature of the position or as requested by Supervisor.
· Adhere to Medicaid and support program rules.
· Must be able to perform duties in accordance with person centered principles.
· Cooperate with the agency, Department of Developmental Disability and Medicaid staff during any inspection and investigation.
Requirements:
- Previous experience working with individuals with developmental disabilities is preferred
- Knowledge of HIPAA regulations and ability to maintain confidentiality
- Strong communication skills and ability to work effectively as part of a team
- Ability to provide patient-centered care with empathy and compassion
- Experience in dementia care is a plus
- Must have a valid driver's license and reliable transportation
- Must have vehicle registration, clean driving record and valid vehicle insurance.
- Successful completion of trainings with the Elizabeth M Boggs center/CDS will be required upon hire.
